a) Continuing Education Plan

Provides clergy and Church House staff $600, for continuing education. In addition, there is a bonus for use of 5% effective January 1, 2004.  NOTE: a limitation has been placed on the amount of benefit for computer hardware/software.  The limit is $1,000 once every three years.

Cost:   Employer contributes $ 500.00 per year.
                        Employee contributes $ 100.00 per year.

b) Dependent Life Insurance

$4,000 life insurance on an employee's spouse and $2,000 on each eligible dependent child.

  • Employee pays full premium.

 

c) Dental Plan
Eligible members may choose from the following:

Basic Dental Plan:

Reimburses an employee for 66.7% of eligible expenses for himself/herself and eligible dependents, to a maximum of $1000 per calendar year, per plan member.

  • Eligible expenses include examinations at least nine full months apart and for children under age 16, six full months apart, cleaning and scaling, x-rays as required, fillings, extractions, periodontics, endodontics and surgical procedures.

OR:

Enhanced Dental Plan:

Reimburses an employee for 75% of eligible expenses for himself/herself and eligible dependents, and 50% reimbursement of expenses for major restorative for (crowns, bridges, dentures) services to a maximum of $1500 per calendar year.

As of January 1, 2002 Executive Committee approved the following changes to the
Diocese of Huron  Benefit Plan:

  • No semi-private coverage
  • $9.00 cap on prescription dispensing fees
  • Increased the drug co-pay to 15%
  • Co-pay, as illustrated in the table below, for Basic and Enhanced Benefits

d) General Synod Pension Plan
A career-average defined benefit plan

  • Employee contributes 4.4% of salary for pension purposes.
  • Employer contributes 8.3% of salary for pension purposes
  • Provides a pension for the employee, or surviving spouse.

e) Group Life Insurance and Accidental Death and Dismemberment - Maritime Life

2½ times annual earnings, minus $10,000 rounded to the next higher $1,000 to a maximum of $190,000.  The $10,000 is covered by the General Synod Self Insured Death Benefit.  (Must have 5 years in Plan).

  • Insurance benefits terminate at retirement or age 65, replaced by a $4,000 Self Insured
  • Death Benefit from General Synod
  • Employee pays full premium.

f) Short Term Disability Plan - Diocese of Huron

Paid by salary source

  • First 4 weeks 100%
  • Weeks 5 and 6, 85% during EI waiting period
  • Top up EI Benefits to 85% to week 17 (119 days)

                                                                                                                                                           
g) Long Term Disability Plan - General Synod for those disabled January 1, 2005 or after

Non Taxable Benefit

  • 60% of predisability earnings
  • Maximum monthly benefit from all sources is 85% of your salary.
  • Waiting period:  119 days.
  • Benefit duration:  to age 65.
  • First two years "own occupation and after that "any occupation".
  • Employers will make contributions to the LTD plan at a rate of 2.2.%.  Employees will make after tax contributions to the old LTD plan of 0.4% of pensionable salary and 0% to the new LTD plan.

h) Major Medical Health Insurance
Eligible members may choose from:

Basic Major Medical (include with basic dental)
Eligible expenses include:

  • Prescription drugs which must be dispensed by a licensed pharmacist, with a 10% co-pay per prescription to maximum out of pocket expenses of $500 per calendar year
  • Out of province/country: coverage limited to 4 weeks while traveling or on vacation
  • In-home private nursing services to a lifetime maximum of $10,000
  • Paramedical services at $500 per calendar year per practitioner, and $300 for
    Massage Therapy (RMT required)
  • Expenses for eye examinations incurred November 1, 2004 and later to a maximum of $50 every 24 months per plan member insured.
  • 15 visit deductible per calendar year for chiropractic expenses incurred January 1, 2005 or late

OR:

Enhanced Major Medical:
Eligible expenses include:

  • As with basic major medical, plus
  • Vision care to a maximum of $150 per every 24 month per

i) Retiring Benefit

Provides lay employees of the Huron Church House with a lump sum payment or at his/her option, in equal installments over five years upon application, a retiring gift provided employee retirees from the diocese..

Benefit:

  • a sum at the rate of $108 per month of service in the Diocese up to the date of retirement or to age 65, whichever occurs first.

Note:

  • if the employee dies in active service, the Retiring Benefit is not paid to the survivor as the Group Life Insurance benefit is paid to the named beneficiary.

Cost:

  • the Diocese contributes to the fund annually.  Annual contribution is based on Parish Apportionment Receipts and is outlined in Canon 34.
