All eligible actively working Clergy and Non-Clergy employees under age 65 have the option to purchase additional term life insurance for themselves, their spouses and dependent children through the group insurance plan. Plan members can apply for this coverage at any time. Coverage can be bought as follows:

Premiums are based on the age of the applicant/plan member on the premium due date so costs will increase over time with attained age. Maritime Life may change premium rates at any time. Premiums are paid 100% by the plan members.

WHEN COVERAGE TERMINATES
Coverage is available for eligible Clergy and Non-Clergy plan members who remain actively working and under age 65 as long as premiums are paid.
Coverage for the plan member, spouse and dependent child will terminate on the earlier of when:
-Plan members terminate their relationship with the Diocese
-Plan members retire
-The Diocese group plan changes providers if no comparable coverage is available
*A plan member may terminate their optional coverage at any time.
HOW TO APPLY
Contact Sheena Hanna as she will be completing the Plan Administrator portion (section 1) which will be sent back to you. THEN complete the Plan Member Portion (section 2 and 3).

Maritime Life will advise plan members in writing if and when their application is approved.
